Schedule: Thursday-Monday 8a-430p, Remote position; only GA residents will be considered.

Summary:

The Population Health (PH) LPN Care Coordinator supports the goals of the Department by identifying and coordinating care for patients meeting criteria for participation in the Remote Patient Monitoring (RPM) program, patients discharged from hospital to home through the Transitional Care Management (TCM) program and patients requiring follow up care with the Mobile Integrated Health (MIH) team. The PH LPN Care Coordinator is responsible for working with patients and families to determine their care needs and coordinates patient-care services, educating patients and their families about their chronic and complex health conditions, and collaborating closely with the MIH Advanced Practice Provider (APP), primary and specialty care teams at Grady Health System to evaluate patient progression towards their goals for optimal outcomes.

Qualifications:

•Two years of college course work, Bachelors degree preferred

•Licensed Practical Nurse (LPN) certification

•Three (3) or more years of experience in clinical or community resource settings, care coordination, and or case management

•Experience with EMR and computerized charting or Epic systems/reports

•Evidence of essential leadership, communication, education, and critical thinking skills, counseling skills and telephonic management skills.

•Ability to work independently and with a team.

•Local knowledge about and connections to community health care and social resources.

•Basic Life Support (BLS) Certification

•Ability to speak a second language, preferably Spanish.
