Job Summary

The Clinical Care Coordinator (C3) proactively identifies and removes barriers that my impede department or systemwide patient flow by serving as a consultant and educator with medical staff regarding patient status, bed placement, payor requirements and utilization of resources. The position (Unit C3) will act as the primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, utilization review to ensure appropriate bed is assigned, patient care needs are met and treatment plans executed at the appropriate levels. Will work collaboratively with all members of the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization.

Responsibilities:

  • Acts as a spoke of the "Hub" by partnering with other C3s and unit staff to facilitate each patient's progression of care at the unit level.

  • Collaborates with the "Hub" to ensure appropriate unit and level of care assignment, and provides "Hub" with visibility into potential unit transfers in a timely manner.

  • Works with Unit Staff and support teams Medical Social Worker (MSW), Clinical Documentation (CDI), etc.) to progress each patient's care toward discharge, and to ensure patient status (anticipated transfers, discharges, location and patient alerts) is accurately reflected in the system.

  • Reviews Physician admission order to validate status and level of care.

  • E-Reviews Physician admission order to validate status and level of care.

  • Rounds with physicians and/or maintains daily communication to discuss patient progression and barriers to discharge.

  • Identifies and assesses patient for readmission, readmission risk and communicates to care team.

  • Coordinates patient's plan of care using patient progression plans and other tools, in conjunction with physicians and nursing for appropriate sequencing of care/interventions.

  • Communicates with patients and families about plan of care and discharge goals, along side MSW and nursing.

Qualifications:

  • Bachelors Degree required

  • 2+ years of RN experience

  • Nurse Case Management experience preferred
